Aktil, Amir and Tahir, here are the homozygous triplets fed by a single placenta

The force of life that bursts with arrogance and imposes itself beyond all obstacles. So it took just half an hour in the delivery room with the background music of ‘With Or Without Yoù by U2 and’ Meglio del cinemà by Fedez to bring to light Aktil, Amir and Tahir who came into the world having to share with each other the nourishment that their mother passed to them through a single placenta. The three homozygous boys were born in Florence at the San Giovanni di Dio Hospital, concentrating the attention, amazement and emotions of the parents, the medical team but also of the public opinion that – darkened by the winds of war – for a moment gave way to the hope of life to come.

One in a hundred thousand

Childbirth faced by mother Valentina is a rare event that occurs in one case every hundred thousand. “Triplets per se does not occur very frequently, but this is an even more particular case because it is a mono-ovular pregnancy”, she explains. Anna Franca Cavaliere, director of the obstetrics and gynecology structures of the San Giovanni Di Dio and Santo Stefano di Prato hospitals. What does this mean in practice? “That in the first days after fertilization the egg divided giving rise to three amniotic sacs, each containing a fetus but in this case the three fetuses shared a single placenta that supported them throughout the pregnancy”.

The risks of a triplet pregnancy

Carrying out a triplet pregnancy involves risks, both for the pregnant woman and for the fetuses: “With only one placenta – continues the gynecologist – the most serious risk is the fetal-fetal transfusion which can compromise the well-being and even the survival of the twins because the distribution of blood is not balanced and therefore it may happen that one twin receives too much and the others too little “. The other risk is that of an extremely premature birth between the twenty-seventh and thirty-second week with all the inevitable possible complications for the unborn child. And for mom? “Above all, he risks preclampsia, but also diabetes and anemia”, replies Cavaliere, specifying that the new mother had complications only in the final phase of pregnancy but fortunately they had no outcome on the unborn children.

The importance of adequate teams and structures

Precisely because of the potential risks and complications that may arise, those who receive a diagnosis of triplets must be carefully followed: “It is necessary to carefully monitor these pregnancies at a higher level center and also to manage and decide when to do them. born must be very accurate and precise aiming to get as far forward as possible “, explains Cavaliere. “It is necessary to choose an advanced level structure with high competence of neonatologists and equipped with instruments and intensive departments for the management of preterm triplets”.

Strong and healthy triplets

The triplets were born in the 32nd week by caesarean section and with a good weight: Akil 1730 gr; Amir 1724 gr and Tahir 1500 grams. “They adapted immediately to life outside the uterus and even if they were breathing alone, they are in the NICU and they are fine. Right from the start they made it clear that they are lively and vital. Maybe tomorrow they will come out of the intensive care”, says the gynecologist adding that the mother brings her milk to the little ones every day.

Five boys and a mother

The parents’ emotion was great: two young boys from Kosovo who have been in Italy since 2008. He, Quamil, 44, and Valentina, 38, already parents of a boy who will turn two in April. “When they gave me the news – says Quamil as he waits for his wife who has gone to visit the children – it was the end of September and it was my birthday. I considered it a gift of life even if we were a little worried because there they said that such a pregnancy could carry risks “. Will you make it in five? “I have a good job and with my wife we have always been optimistic and positive: these three little ones are a gift that life has given us and we want to enjoy them”.

The emotion of the medical team

Gynecologists, obstetricians, neonatologists, anesthetists and nurses shared a unique and exciting experience also because the birth of mother Valentina came after the news of another diagnosis of trigeminal monochorionic pregnancy made at the hospital in Prato. The birth then took place at the Careggi hospital: “It is an incredible coincidence – the gynecologist says with emotion – because in itself the trigeminal birth is a rare event and a gynecologist can never see it in all its career but discovering that two women at the same time have successfully carried out a pregnancy of this type has given us a very strong message of life especially in these days of war. In twenty years of profession for me it was the first time I did to be born three identical twins supported by the same placenta and it was a very strong emotion for which I am grateful to life “.
