Anti Covid treatment, drug blocked by the FDA

The US Food and Drug Administration rejected the application for authorization to use a generic drug with anti-inflammatory properties (cheap and widely available on the market), which until now has been indicated as a possible anti Covid treatment. Specifically, it is fluvoxamine maleate, an antidepressant which belongs to a class of selective serotonin reuptake inhibitors (SSRIs).

Being an easily available product, in fact its use could have represented a turning point for the sick and their diagnosis, but the authorities have decided to block it.

The use of the antidepressant to treat Covid has been blocked

The American FDA has stuck Monday 16 May 2022 the authorization for the emergency use of the fluvoxamine maleate as an anti Covid treatment.

The question to formally and officially recognize the antidepressant as effective cure against Coronavirus was presented in December 2021 by Professor David R Boulware of the University of Minnesota, after several studies on the treatment used to prevent disease progression in affected adults aged 24 years and older, positive for Covid-19, in an outpatient setting. .

According to the results obtained by the researchers, the use of fluvoxamine it would have reduced hospitalizations by almost 30%also reducing cases of progression of the virus which reduced the number of emergency room visits (and waiting hours) by more than six hours.

However, after analyzing the data provided, the US regulatory agency noted that there have been some uncertainties about the analysis. Hence, the refusal to authorize fluvoxamine as an emergency treatment for Covid. Specifically, the FDA said the data are inadequate to conclude that the drug was effective in preventing progression to severe disease and / or hospitalization in out-of-hospital patients.

What are the drugs used to treat Covid today

Having to deal with a virus never “encountered before”, to one cure for the treatment of Covid it has arrived over time, even if studies and analyzes by scientists have never stopped, and still continue now.

To date, the main defense weapon against this disease remains the vaccine. Vaccination, in fact, also activates the T cells that prepare the immune system to respond to further exposure to SARS-CoV-2 (here the study on the subjects who are more likely to contract Covid several times).

The vaccine, therefore, does not introduce the actual virus into the cells of those who are vaccinated, but only the genetic information that the cell needs to build copies of the Spike protein. If, at a later stage, the vaccinated person comes into contact with the virus again, their immune system will recognize the virus and be ready to fight it.

As for instead drugs that can be used to treat the disease (and to prevent this from degenerating), AIFA – the Italian Medicines Agency – has prepared cards (available here) that make explicit the therapeutic guidelines within which it is possible to foresee a controlled and safe use of the drugs used in this context. emergency. The cards clearly report the evidence of efficacy and safety available today, as well as the interactions and recommended methods of use in Covid patients.

Treatments target patients symptomatic, and distinguish between home and hospital care. With regard to home management, Paracetamol or NSAIDs they can be used in case of fever or joint or muscle pain (unless there is a clear contraindication to use), while other symptomatic drugs can be used on clinical judgment.

Three have also recently been made available antivirals (remdesivir, nirmatrelvir / ritonavir and molnupiravir) for the treatment of Covid-positive adults who do not require supplemental oxygen therapy and who are at increased risk of progression to severe forms of the virus. In order to use them, specifies the AIFA circular, the patient must present a mild to moderate form and at least one of the risk factors associated with evolution into severe disease.

Currently, therefore, therapies, both with monoclonal antibodies and with antivirals, are indicated for subjects with:

  • Recent onset mild to moderate Covid-19;
  • not hospitalized and not on oxygen therapy;
  • risk factors for the development of severe forms of the disease.

The European Commission, on the advice of the European Medicines Agency (EMA), has authorized the following medicines containing monoclonal antibodiesalone or in combination, against the spike protein of the SARS-CoV-2 virus:

  • casirivimab-imdevimab association called Ronapreve (of the pharmaceutical company Regeneron / Roche) for the treatment and prevention of COVID-19;
  • regdanvimab called Regkirona (of the pharmaceutical company Celltrion Healthcare Hungary Kft) for the treatment of COVID-19;
  • sotrovimab named Xevudy (from GSK) for the treatment of COVID-19.

Different speech, however, for the hospitalized and positive for Covid. Based on the numerous scientific evidence, explains the Agency, in the last months of the pandemic for the treatment of patients hospitalized with Covid, the current standard of care is represented by the use of corticosteroids and heparin.

Anti Covid treatments and fake news: how to recognize them

With the proliferation – especially on social networks – of false information and conspiracy theories supported by conspiracy theorists, during the pandemic (even when the vaccine was approved) the fake news relating to treatments for the treatment of Covid do it yourself, natural and without any scientific basis.

First of all, we must specify that no: antiviral drugs do not prevent new Coronavirus infection. There is no evidence of their preventive action. Some specific antivirals are useful in the post-exposure and treatment phase to avoid disease progression in those at increased risk.

Some drugs marketed for other indications are sometimes used in scientific studies, even in the absence of a specific therapeutic indication for Covid, based on evidence to be refuted. Precisely for this reason the health authorities recommend avoid empirical use of antibiotics without consulting your doctor first, and outside of cases in which bacterial infection has been demonstrated by a microbiological examination. Antibiotics, therefore, should not be used for the prevention or treatment of Coronavirus without any logic or without being followed by a professional. When in doubt, it is better not to rely on hearsay alone, especially when it comes to health.
