Dr. Cristina Bianchi, primary care physician in the territorial area of the Municipalities of Cesiomaggiore, San Gregorio nelle Alpi, Santa Giustina, Sedico and Sospirolo, will finish her assignment on June 30 (last day of service). General medical assistance paid by the Regional Health Service will be ensured by Dr. Anna Caldart who will carry out, from 1 July, the activity of primary care doctor – with temporary appointment – at the outpatient clinic located in via Cal Del Vento n. 9, headquarters of the group medicine of Santa Giustina. Bianchi’s clients will be automatically assigned to Caldart; it will therefore not be necessary for them to go to the Ulss 1 Dolomiti registry office to make the choice of the new doctor.
However, there remains the possibility of making a different choice between doctors in the area of residence who have available places, by sending an email (not certified e-mail) to the address [email protected] with the following subject, “Termination dr.ssa Bianchi “, indicating name, surname, place, date of birth, residential address, tax code of the patient and the name of the primary care doctor chosen (it is suggested to indicate more than one name and up to a maximum of three). A copy of the identity document and the paper health card of the assisted person issued by the ULSS must be attached to the e-mail. At the end of the operation, an e-mail confirming the assignment of the doctor will be sent with an attached copy of the paper health card.
