Almost half of the people who ask for the recognition of civil invalidity they are affected by mental illnesses and cancer. This is one of the data that emerged from the report by the general coordinator of the INPS Raffaele Migliorini, who spoke during the conference “Social medicine in evolution: between innovation, new needs and protection of disability”, organized by the Mesit Foundation in collaboration with the network editorial PreSa – Health Prevention, the Ceis of the University of Rome Tor Vergata, the Crispel of the University of Roma Tre with the non-conditioning support of Sanofi.
«Studying the state of health of the community and analyzing it in the light of social, demographic, scientific, technological and economic variables that are involved in it arouses great curiosity and enthusiasm in me ». This was stated by Marcella Marletta, president of Aistom – councilor of the Federation of Cancer Volunteer Associations and coordinator of the Scientific Committee of the Mesit Foundation, speaking at the conference.
«The Foundation, marrying the concept of One Health evolved into that of Planetary Health, demonstrates that it has fully grasped the new health needs on which our future and that of our children will be played out. In this context, I will try to give my maximum contribution as well as in that relating to the promotion of Italian technological and scientific competitiveness – he added – The starting idea is that the development of new drugs and medical devices can represent a truly significant breakthrough in patient care. Objective that the Mesit Foundation intends to pursue through the forthcoming constitution of a working group dedicated to the theme of competitiveness and innovation, made up of the leading experts in the sector. A working group that will contribute to the definition of the new governance of the pharmaceutical and medical devices sectorwith the ambition of becoming a true point of reference on the subject and a support tool and an intermediary with the productive world for the highest national institutions ».
«The pandemic is giving us health that goes on a double track: on the one hand, the track of innovation, advanced research, innovative therapies, as demonstrated by the experience of vaccines against Covid-19, which allowed us in a very short time to stem the pandemic tsunami. On the other hand, the issue of the protection of rights, especially the disability protections. The challenge that awaits us today is to understand how much innovation and timely access to it can reduce the impact on disability, especially with regard to chronically ill – observed Marco Trabucco Aurilio, member of the Mesit Foundation and PreSa Scientific Coordinator – Today we talk about new needs and with the pandemic new achievements have also arrived. We face a huge challenge and we cannot afford to waste time ».
The commitment of the professionals also aims to give new impetus to a cultural revolution of the health system, starting from school. In this sense, the PreSa network has long been committed to animating a debate that sees social medicine and health education within schools.
Civil invalidity: who asks for it in Italy?
In 2021, almost 42% of all civil invalidity reports drawn up in Italy were related to psychic pathologies and al cancer. This is one of the data that emerged from the report of the general medical examiner coordinator of INPS Raffaele Migliorini. In the course of 2021, 174,315 civil invalidity reports concerned i tumors (it 20.48%), 181,641 the mental illness (21.34%), 137,197 (equal to 16.12%) le diseases of the nervous system117,134 (13.76%) on cardiovascular diseases and 101,363 (11.91%) le diseases of the musculoskeletal system. Among cancers, the greatest impact on disability certifications lies with breast one with 13.53%, followed by the colorectal one (6.31%), of the lungs (6.11%), of prostate (3.74%). Almost a third of the minutes also refer to tumors already in the metastatic phase. «Whenever there is a delay in the recognition of these pathologies, it is evident that the reverberation can be severe or very serious. So there could be an impact, ”said Migliorini, who nevertheless underlined some positive aspects that emerged from the pandemic.
“After the virus appeared, we realized that we could not have summoned such fragile people to visit exposing them to a high risk of infection. So we avoided having them reach the clinics and sent them to send us the medical reports to give them a medico-legal translation. This has led the institute to satisfy the requests of these patients, avoiding the possibility of contagion ».
«Access to the highest possible level of health is an extremely important objective: a challenge for the future of the new generations which requires the participation of many other strategic sectors besides healthcare. Following the “One Health” logic we must hope a return to health planning and programmingsupported by the HTA tool with specific reference to real needs », explained Francesco Saverio Mennini, professor at the Faculty of Economics of the University of Rome“ Tor Vergata ”and President of SiHTA.
«It is essential to produce evidence, following a multidomain and multidisciplinary methodology that allows, in addition to the correct assessment of technologies, also monitoring, a fundamental element for the purposes of correct planning and programming. In the last 15-20 years, emergency tools have been used without proper planning while today, after the period of the pandemic, the conditions seem to have been created to guarantee a “new Renaissance” of our health system which cannot ignore efficient programming and planning (also and above all organizational-managerial) – Mennini noted – It is necessary to fill the structural and technological gap that unfortunately characterizes our territorycon differences especially between North and South and that also affects the health mobility, generating incremental costs, inefficiencies and, above all, inequity in accessing care. To do this, it is necessary to reconsider the value of technologies on the basis of the real utility that derives from their use. Not only from the point of view of the national health system, but of the country system as a whole ».
