Heart murmur, that noise often in children is “innocent”


Heart murmur is a small turbulence, very common in children, due to the acceleration of blood flow through the structures of the heart, arteries and veins. The flow, which is normally silent, can be perceived as a rustle. This particular noise that the doctor’s ear perceives is called a “breath”.

They can be divided into 2 groups: “organic” and “innocent”. The latter are very frequent between 1 and 14 years (between 50 and 70%) and are not the symptom of a heart disease. The organic heart murmur, on the other hand, is that noise which, upon auscultation of the chest, is equivalent to a real cardiac anomaly in the structure of the heart.

For the pediatrician the problem to be faced is therefore to safely discriminate an innocent breath from an organic breath. In some conditions there may be a sufficiently identifiable difference, that is, a very intense and strong breath is certainly of an organic nature. However, there are cardiac pathologies whose murmur is not particularly pronounced. Hence the intervention of the specialist to subject the child to tests such as electrocardiogram (ECG) and echocardiography useful to clear the field of doubt. Once it is established that the murmur is functional in nature, and the specialist can ascertain it in 99.99% of cases, the heart itself is normal.

Professor Bruno Marino, full professor of Pediatrics at La Sapienza University explains: “If the problem from the health point of view is not serious, the diagnosis of heart murmur generates a lot of anxiety in families, a condition that pediatrics must take care of, because it can affect and condition the child’s life and development. For this reason, the pediatrician can request a specialist visit with an electrocardiogram, to be done without urgency. The innocent breath it does not cause symptoms, it does not require therapy, and if you have a heart murmur, you don’t need to limit physical activity or take any other special precautions. Sometimes you no longer hear with growth while sometimes you continue to listen throughout your life “.
