Patients discharged with saturation at 92% and still infected: Catania doctors in revolt

by admin

Doctors from Catania in turmoil over the latest provision of the manager of the regional health department regarding the criteria for the discharge of Covid patients. In the note, sent to all hospitals and local companies, Mario La Rocca writes that due to the increase in cases of infection it is necessary to “ensure maximum appropriateness in the choice of the care setting, ensure a correct turnover of hospitalized subjects so as not to overload the structures hospitalization and at the same time guaranteeing maximum safety for patients “. And he continues: «The councilorship has sent the document of the regional CTS on the subject of the domiciliary of patient care. Among the indications contained there were specific criteria, defined in a recent document of the national agency, to be applied for the discharge of Covid hospitalized patients even in the event of persistence of positivity, but in sufficiently stable clinical conditions to be discharged at home “. Among the criteria for resignation, the manager reported apyrexia for at least 48 hours, saturation starting from 92%, not in oxygen therapy, hemodicamically stable. La Rocca points out that these people who are sent home can be taken care of by the Usca with adequate monitoring of vital parameters, guaranteeing prompt availability to patients … ».

In short, patients who in the past would have remained hospitalized are now sent home under the supervision of the continuity of care doctors.. But it is not as simple as the councilorship says. And in fact there is a certain turmoil in the Covid departments and some doctors argue that an oxygen saturation of 92% is equivalent to an oxygen pressure in the arterial blood of about 60 millimeters of mercury. This means that the patient is in respiratory failure and deserving of oxygen administration. Furthermore, the discharged patient is still highly infectious and therefore it is not known whether sent home can be a vehicle of contagion for the whole family. Despite the protest of the doctors it seems that the department has sent the first inspectors to the hospitals to verify the application of the discharge criteria that have so far been disregarded by the doctors both to protect patients and to avoid any legal disputes.
While the councilorship tries everything to prove that it is able to contain a contagion that is growing more and more – thanks to the fact that many citizens still do not want to get vaccinated – the emergency is growing in hospitals. Yesterday, another patient from 1972, aged only 49, was transferred from the Covid Pulmonary Clinic of Cannizzaro to the reanimation of Garibaldi. Chief physician Sandro Distefano said he is in very serious condition. It is obviously not vaccinated.

By his colleague from Infectious Diseases, Carmelo Iacobello another news arrives, that the positive subjects among those vaccinated only with a first dose are also increasing: «This is what we have been noticing for a few days. Even partially vaccinated people, those with a single dose, become infected and show symptoms. Obviously they don’t develop severe symptoms, but some of them have to be hospitalized ». «At the moment – continues Iacobello – all the serious patients we have in the ward are unvaccinated. As for the capacity of the wards, we are defending ourselves by dosing new hospitalizations and discharges. But it is clear that if the situation continues like this, then we will have to review all the parameters of care and increase the beds, which we are about to do. In brener, the medical department of prof. Malatino who right now is trying to transfer the ordinary patients he still has in the ward. It is clear that the persistence of the pandemic on these numbers will make assistance to non-Covid patients more difficult ».

In terms of the numbers of unvaccinated citizens, the march continues at the rate of just over two thousand first doses per day, which made it possible in just over a week to lower the number of citizens still to be vaccinated from 344 thousand to about 330 thousand. Of course, if the immunization trend continues this it will take no less than six months to vaccinate all people and achieve herd immunity. In short, there is little to be happy. A separate chapter concerns the measures taken by the health authorities against doctors and nurses who by choice do not want to vaccinate. The Order is still waiting to know the names of the doctors who did not respond to the good-natured letter sent by the ASP to get vaccinated. The same would be done for nurses. It would be useful to know what time frame the ASP has to communicate the lists and provide for the suspension which will last until 31 December according to the provisions of the government.
