World premiere Padua cardiologist-urologist tandem intervention – Medicine

(ANSA) – PADUA, 11 JUN – A multiple combined operation of kidney tumor removal with robotic surgery with arteriovenous Ecmo implant was performed for the first time in the world at the hospital of Padua to support the heart during urological intervention and implantation of an artificial ventricle with an innovative micro-invasive technique to minimize the stigmata of the intervention to the patient undergoing urological surgery six days earlier. The patient is a 70-year-old with severe heart failure.

The man was recently hospitalized in the Cardiac Surgery of the Gallucci Center, director Prof. Gino Gerosa, for a severe worsening of the clinical picture. He had been undergoing treatment for severe heart failure for ten years. Clinicians evaluated the hypothesis of a heart transplant or a left ventricular assisted Vad implant. After a CT scan, a tumor was found in his right kidney. The case was immediately studied by a multidisciplinary team where radiologists, urologists, cardiac surgeons and anesthetists met to define the optimal therapeutic approach.

Renal carcinoma had to be removed as soon as possible to avoid metastatic dissemination but at the same time the severe heart failure made urological intervention prohibitive due to the very high risk of mortality. The cardiac surgeons have implanted the Ecmo (mechanical assistance system to the circulation) which has supported the heart allowing first of all the robotic urological intervention; after which he intervened on the heart by implanting the Vad. (HANDLE).
