To speed up the procedures and shorten the lines at the vaccination sites, it is useful to answer the questions at home
TREVISO. To speed up the vaccination, you must arrive at the vaccination points with the completed and signed anamnesis form. In recent days, long lines have also formed to compile anamnesis which, especially for elderly people, is not so immediate. Hence, the Usl have equipped themselves by providing the form. In addition, the booking phase via internet for vaccinations in local health authorities in Veneto will start from 1 April, through a single interface.
The dedicated page will be accessible from the site of each Usl or from the regional vaccination portal. Once you have chosen your health company, the user enters his tax code.

How to download the form. Click on the symbol you see at the top right of the form below, the one with the arrow inside a square. A new page opens where you see the form in its entirety: always at the top right you see another symbol, that of a downward arrow: click on it and the pdf with the form will be downloaded to your computer desktop: print, fill in and take with you.
It will also be possible to book in 1400 Venetian pharmacies. Pharmacists will help people book and fill out the medical history sheet. There will be no cost for the citizen, the expense will be covered by the Veneto Region.
