The request must include these data: surname, name, date of birth, tax code, municipality of residence, mobile number. The vaccination certificate relating to the first dose must be attached to the request. At this point, explains Ulss 1, the vaccination team of the Prevention Department will process the request within 7 days, communicating the appointment via email or mobile phone.
