Oriana Fullin takes stock of her current health conditions and forecasts: “The parameters are all good, they are back to normal, I will not have to undergo chemotherapy because what has been done, especially at the level of surgical removal of the tumor, I do not will expose you to relapses. There are no words to thank all these professionals who saved me from a disease that would have left me no way out.

From now on I will certainly have to undergo periodic checks that I will return to do in Belluno. But they will be routine checks. The prognosis was disastrous, mine was a healthy liver, that is, not compromised either by hepatitis viruses or by autoimmune diseases, on which a rare form of cancer developed. I had no typical symptoms or signs that would indicate its presence when I began to feel sick. And even with the great CT diagnostics it was not seen immediately.

But by the time the picture became clear, the hepatocellular carcinoma was already in an advanced state. And in mid-May it was found to be inoperable. One can only imagine the days of anguish I have endured. Until a glimmer of hope opened with the chief surgeon of Feltre, Montin, who was optimistic about the success of the surgery. He was right ».

The intervention

An entire consortium of Ulss Dolomiti specialists dealt with this case judged to be inoperable for diagnostic evaluation. A little light of hope thus became an intense light when, thanks to the sharing of skills and professionalism, it is highlighted by the Ulss management, the team devised a strategy that included chemotherapy treatment, interventional radiology and liver surgery.

“The tumor, in fact, could only be removed by leaving a very small segment of the liver that would have caused death from hepatic insufficiency”, explains Valter Vincenzi, of the hepatology clinic of Medicina di Belluno.

After the diagnosis and staging of the disease performed by the radiology of Belluno, the patient was entrusted to the oncology of the head physician Fable Zustovich, who subjected the patient to chemotherapy for mass reduction. However, at the end of the treatments, the tumor was still too large to be removed.

Riccardo Berletti, director of Radiology of Feltre, then intervened, who subjected the patient to radiological angiographic treatment to embolize the vascular structures of the liver affected by the disease, exploiting the regenerative capacity of the residual healthy liver. The intervention of Umberto Montin, director of Chirurgia Di Feltre, with consolidated experience in the treatment of liver and pancreatic cancers, was therefore made possible.

“The margins of operability were small due to the extent of the disease and the few remaining healthy liver,” explains Montin. “I subjected the patient to the removal of six out of eight liver segments with maniacal attention to preserve the vascularity of the healthy residual liver. The important intervention, which lasted 5 hours, was also possible thanks to the contribution of the anesthesia team led by the head physician Federico Innocente and the contribution of the director of pathological anatomy Duilio Della Libera who gave a histological assessment in real time on the quality of the regenerated liver and the absence of lymph nodes affected by the disease “.
