Enhancing Care for Complex Chronic Patients in Cuenca: The Horizon 2025 Health Plan

Cuenca, April 14, 2024.- The professionals of the Internal Medicine Service of the Virgen de la Luz Hospital in Cuenca have been updated on the regional strategy for the care of complex chronic patients being developed by the Government of Castilla-La Mancha as part of the Horizon 2025 Health Plan. This strategy aims to provide a global and comprehensive approach to patients with chronic diseases.

During a clinical session, the professionals learned about measures aimed at improving the resolution capacity of Primary Care, enhancing home care for patients, and promoting nursing involvement in caring for at-risk patients. The creation of Care Continuity Units for Complex Chronic Patients (UCAPI) has been highlighted, providing personalized care to individuals with multiple conditions and high clinical complexity since 2017 in the Cuenca area.

The goal of these units is to offer quality care through collaborative efforts between Primary Care professionals and the UCAPI multidisciplinary team to ensure early and efficient intervention for patients, ultimately allowing them to remain in their environment.

Director of Care Continuity and Management Care, Begoña Fernández Valverde, emphasized the importance of integrating Primary and Hospital levels in managing individual cases. The strategy aims to reduce hospital admissions by providing proactive monitoring and shared care between primary care teams and UCAPI professionals.

The UCAPI in Cuenca has provided over 2,600 consultations in 2023, including technical interventions such as medication administration and ultrasound. The unit has also resumed monitoring activity for patients with high complexity in the hospital, offering personalized support and preparing them for discharge.

The implementation of the regional strategy for complex chronic patients in Cuenca demonstrates a commitment to delivering comprehensive and integrated care to improve patient outcomes and reduce hospital admissions. The collaborative efforts of healthcare professionals aim to provide personalized and effective care to individuals with chronic conditions.
