This today is requiring changes in training, organizational and professional policy paradigms.

“The health needs of the population have changed, but also the organizational needs and expectations of the nurses themselves: graduate nurses with PhDs, first and second level professionalizing and specific master’s degrees for high skills. The functions of Nurse Managers have also been implemented in the management area, prepared and trained in the specific governance of nursing care and in safeguarding the health system of which these professionals are increasingly an expression of guarantee, safety and quality of care” – points out Dr. Marika Colombi (in the picture), Welfare Director of the Local Health Unit and University Hospital of Ferrara.Following a recent amendment to the regional law on the organization and financing of healthcare companies, the role of Healthcare Director was established with the task of participating in the definition of company strategies, in synergy with all the members of the Strategic Management, promoting the development of health professions (nurses, midwives, technical health professions, rehabilitation and prevention), with the aim of an increasingly effective and integrated health response.

The nurse also fulfills his role as a health educator in families, schools, workplaces, public health facilities, correctional facilities, long-term and home care, hospitals and other community settings, nurses are protagonists in building a culture of health.

The organizational models implemented, at hospital and territorial level, are based on the construction of proximity networks, resulting in a shift of care settings from traditional places of care, such as hospitals, towards more sustainable and accessible territorial structures that can promote socio-health integration and the continuity of the paths, determining the development of integrated assistance projects between the two Ferrara healthcare companies”.

NURSE AT THE “SEE AND TREAT” CLINIC. As part of the reorganization of the emergency network in the province of Ferrara, various nursing-based organizational models have been introduced and are being implemented. First of all, within the Emergency Department, the model “See and Treat” or more simply “Look and Treat”: a clinic managed by nurses, where access can be achieved through triage, and on the basis of some previously identified and agreed protocols, different types of procedures and services can be activated, from the detection of parameters to medication, to the management of medical devices. The protocols identified are standardized and allow the nurse to carry out the process from taking charge to the conclusion of the process. Currently the See and Treat model is already active in the emergency rooms of the Delta, Cento and at the Cona hospital. Another important project that was developed in the Cento and Delta emergency rooms, and which was already present in Cona, is the “Fast Track”: in particular the orthopedic one. This organizational model allows you to reduce waiting times.

INFERMIERE FLOW MANAGER E BED MANAGER. In this context, the figures of the Flow Managerresponsible for the flows, and the Bed Managerresponsible for the management of beds.

The Flow manager is present in the emergency rooms of the Delta, Cento and Cona hospitals (where it started in April this year) and is part of a broader project on a provincial basis, aimed at optimizing the emergency emergency service. Together with other elements, it has the aim of giving a timely and effective response to patients with low complexity emergencies and providing them with personalized assistance.

The Bed Manager is a figure who liaises with the Flow Manager nurses of the various EDs, with the task of identifying the best possible hospitalization solution for the patient, also taking into account the situation of the beds in the various provincial structures. The provincial Bed Manager is a new figure, foreseen in the Ferrara area, who is in continuous relationship with the directors of the EDs, with the medical managements and with the managers of the various departments, including the surgical areas, which can accommodate hospitalized patients .

THE ASSISTANCE AND URGENCY CENTER (CAU). Il CAU it is part of the plan to improve the emergency room network and more generally the local healthcare system; represents one of the most important actions of the reorganization plan of the emergency emergency network in Emilia Romagna. The territorial CAUs welcome patients, both adults and pediatrics, who have urgent problems with low clinical care complexity within the primary care care network.

THE CASE MANAGER MEDICAL AREA AND SURGICAL AREA. Its role is aimed at improving the appropriateness of hospitalizations, the quality of care and the care of patients with a view to hospital-territory continuity depending on the specialist hospitalization setting.

This figure, developed through an inter-company project, involves taking care of the patient and his family through more or less complex care paths, depending on the case, to facilitate the return to home or placement in long-term care facilities and/or or rehabilitation.

THE FAMILY AND COMMUNITY NURSE is the professional responsible for nursing processes in family and community settings, possessing specialist knowledge and skills in the nursing area of ​​Primary Care and Public Health. It is committed to health promotion, prevention and participatory management of individual, family and community health processes, operating within the Primary Health Care system. It can be activated directly by the citizen, by the general practitioner or by the pediatrician of free choice, by hospital departments, by the emergency room, by the local emergency 118, by the community hospital, by home care, by the CAU, by the psychologist communities, from voluntary associations or from patients, from continuity of care centers, from pharmacies and from the neighbourhood.

THE PROVINCIAL DIALYSIS NURSE is part of the integrated paths of the two Ferrara healthcare companies with the aim of creating synergies and implementing homogeneous management models of the healthcare activities present within the Decentralized Dialysis Centers (CAL) “Bergami 1” and “Bergami 2”, at the San Rocco Community House, through the integration of the two CALs and the identification of a single integrated nursing team at provincial level.

THE NURSE OF THE TERRITORIAL OPERATIONS CENTER (COT). In the COT, the nurse takes on a key role in coordinating the care of the person and liaising between services and professionals in order to ensure continuity, accessibility and integration of health and social care.
