Starting from this month, the Oncogeriatrics Clinic of the Asl di Biella moved from the hospital to be more “present” in the area. Doctor Federica Francisetti, specialist in Geriatrics and recently inserted in the Presidio Health District, will be responsible for carrying out oncogeriatric visits in the Health Homes present in the area, in collaboration with the Complex Structure of Oncology and the Simple Structure of Palliative Care of the Company. Elderly patients who need this kind of services, for a few days can therefore be taken care of in a context close to their home and followed in synergy with company specialists, territorial specialists, local nurses and social services based on individual clinical needs. and welfare. Another important initiative medicine project will take place by the end of the summer, addressed to fragile patients and called Protected Discharge and Pharmacological Reconciliation. Fragile elderly users in need of important pharmacological support, discharged from the hospital in a “protected” manner, will be able to benefit from geriatric and nursing consultancy aimed at reshaping the therapy also through the use of telemedicine tools, improving the appropriateness of the path of care and making it more easily usable in the domestic context. The reshaping of drug intake schedules and the transition from “solid” therapy to “liquid” therapy for patients with swallowing problems are just some examples of what will be proposed within the project. Furthermore, with the help of telemedicine, in a team with local nurses and general practitioners, telemonitoring will be carried out to try to catch any danger signals in the bud by reducing re-entry to the hospital. The details on the functioning of the service will be communicated to users in the coming weeks, once defined. The ongoing demographic evolution and the consequent change in the needs of the population in the health sector, with a growing share of the elderly and people suffering from chronic diseases, make it necessary to redesign the structural and organizational network of services, especially with a view to strengthening the area of territorial assistance. In this sense, the methods of providing health and social and health services enabled by telemedicine contribute to ensuring various improvements in personal care: equity in access to care in geographically remote areas, support for the management of chronic conditions, access channel to specialties and a better continuity of care through multidisciplinary comparison (as per “National guidelines for telemedicine” of 20 February 2014) especially for elderly and frail patients and therefore with difficulty in accessing services.
