The Oncological General Surgery team of the John Paul II Cancer Institute of Bari led by Doctor Aurelio Costa dealt with a “rare case of Oncological Surgery”: the complete removal of a liposarcoma originating from the retroperitoneum, en bloc with kidney, colon, pancreas, diaphragm and uterine appendages, which had generated a voluminous abdominal mass of over 50 kilograms.

The General Director of the Hospital explained the operation that Doctor Aurelio Costa and his team had to face and the delicacy with which they had to manage it: «It is a rare case of oncological surgery, which requires high experience in clinical-surgical management, due to the extension of the tumor mass which completely occupied the abdomen, infiltrating the left kidney, the left colon, the left pancreas, the uterine appendages and partially the diaphragm. The complete and intact removal in a single block, in a 4-hour operation, represented an indispensable and decisive element for a higher possibility of recovery from the neoplasm.”

Doctor Aurelio Costa explained the situation in which the woman found herself: «The patient, a 66-year-old woman, had noticed for about a year an increase in the volume of the abdomen associated with loss of appetite and consequent weight loss, the appearance of of intestinal transit and constant anemia with the progressive loss of walking capacity due to the bulking action exerted by the mass. The precarious initial clinical conditions and the complexity of the surgical procedure did not influence in any way the post-operative course which proved to be regular and without complications with the patient being discharged two weeks after the operation in extremely improved general conditions with recovery complete with its functional motor and digestive autonomy”

«Retroperitoneal sarcomas are rare tumors and constitute approximately 10-15% of soft tissue sarcomas, i.e. only 1% of all tumors. These are often voluminous lesions, on average 10 cm in size, but which can reach considerable extensions, as in our experience described, even to the point of occupying the entire abdomen. Surgical treatment of complete removal represents the standard treatment, and in sarcomas of the retroperitoneum in particular, the quality of the initial surgery is correlated with long-term survival. Due to the particular anatomical site of origin, it is often technically complex to obtain a surgical resection with wide safety margins for this reason it is necessary to associate, with the removal of the mass, all the anatomical structures involved by the neoplasm in order to guarantee the radical removal of the neoplasm”.
